There is some blood, plenty of injuries, and the occasional decapitation. People are more so defeated than killed. And the anime replaces any loss of limbs seen in the manga with broken arms / legs.

He survived by a mysterious treatment that left him perceiving the whole world as made of rotting flesh. A girl named Saya is the only human thing left. Pretty gross, violent, even some torture. What makes it fun to play is to find out about the mysterious treatment and what Saya really is. And h-scenes.
